The standard Medical Information Form is required to be filled out and signed by parents each year.
Immunizations
Immunizations are required by state law for all students attending public school. A copy of the immunization record is kept in each students health file. Families that object to immunizations for medical or religious reasons may return an objections statement (found on the back of the immunization record form) each time new immunizations are needed. This form must be notarized.
Individual Health Plans
Health Plans are used at NCA to notify teachers, specialists, and bus drivers of potentially life-threatening health conditions. Health Plans are renewed annually and updated at the beginning of each school year. If your child has a health condition that the nurse’s office should be aware of, please print and complete a plan and bring it to the office.

MEADA Coalition
Methamphetamine Education and Drug Awareness (MEADA) Coalition of McLeod County was created to educate communities on the dangers of drugs with a focus on methamphetamine.

Medications at School
If your student needs to take medication during school hours, please take a moment to review the following guidelines to ensure proper administration.
Prescription
- Permission Form must be completed by the parent and the student’s physician.
- Permission Form must be renewed annually.
- Container must be labeled by the pharmacy with the medication name, dose, and doctor’s name.
- Medication should be brought to the health office by the parent.
Non-prescription Medication
- Original unopened container.
- Sample size ok.
- Plastic bag or mixed medication will be returned.
- Will be given by manufacturer’s instructions unless otherwise authorized by physician.
- No stock medications. Each student must have their own container.
- Permission Form completed and signed by parent and renewed annually.
- If medication is left at the end of the year, it is the responsibility of the parent to pick it up within 2 weeks of school ending. One letter or phone call with a reminder will be given.
- All medications not picked up will be disposed of by the nursing staff.
